Our office is located on the corner of Chapman Ave. and Kraemer Blvd. next to the IHOP on the corner. Go in between the two big buildings behind the IHOP, drive straight to the back wall and our office opens out to the parking lot. Do NOT go inside the main building. We are located inside Placentia Chiropractic.

Dr. Mortensen provides DOT certified physicals to drivers from all part of Southern California. He is even able to perform DOT Physical testing for other states as well. Following the FMSCA guidelines mandated by federal regulations you can be rest assured that your DOT physical is valid, certified and clear for driving either interstate or intrastate.
